Chaplain's Visitation Log

In some long-term care settings it would be helpful to the staff of the facility, especially the activity director, if a log were kept documenting the various resources and activities that the volunteer has shared with which residents.  A log like this would also be helpful to the minister in sharing the practical aspects of his or her ministry with supporters.  For an example of a document developed and proven in the field and used for such a purpose in nursing home ministry, click here for a .pdf file of the Chaplain's Visitation Log.
